If you have any quality of care or safety concerns about St. Alexius Medical Center that you feel have not been adequately addressed, please call St. Alexius Administration at 701-530-7600. As an accredited facility by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO) you also have the option of notifying the Joint Commission. 3. Send completed form by e-mail, fax or by mail  to Health Information Management. If you are emailing the form, you must scan the completed document and attach it to your email. The form is not interactive.
