At St. Alexius Rehabilitation Center we emphasize community reintegration for our patients. Through daily practice, ongoing education and peer interaction, patients at St. Alexius Rehabilitation Center acquire the confidence to pursue a life of active participation within the family, the community, the outdoors and the workforce.
Community reintegration is an important opportunity for each patient to establish him/herself back into the community and determine his/her level of functioning prior to discharge home. Community reintegration includes community outings, guidance for home modifications, vehicle training, transportation needs, community resource education, equipment training and use, and recreational training to encourage patients to again enjoy previous interests or explore new ones.
